Medal was instituted by Ministry of Defence on 15 May 1991.
Medal is awarded to Senior Officers for distinguished service and excellent command of Army, Navy or Air Force formations, to foreign Officers for distinguished service rendered to the Hellenic Armed Forces or the Hellenic Republic, and to Greek citizens and foreigners for meritorious actions rendered to the Hellenic Armed Forces or the Greek nation.
Medal is awarded in two classes: Commendation Star for Merit and Honour (Διαμνημόνευση Αστέρος Αξίας και Τιμής) and Commendation Medal Of Merit And Honour (Διαμνημόνευση Αξίας και Τιμής).
