- Medal instituted by Decree 4118 of 14 March 1868.
- Medal awarded to the marines of the fleet, who on 19 February 1868, forced the passage of Humaita in southern Paraguay, at the junction of the Uruguay and Paraguay rivers, after a thirteen months siege of the fortress.
- Medal awarded in three classes: gold, silver and bronze.
- Passage of Humaitá - was an operation of riverine warfare during the Paraguayan War.
